Every incident becomes a single case file: the evidence, the causal chain, the corrective actions, and the sign-off, with every finding cited to the evidence behind it.
Overview
Intero is an incident investigator, not a system of record. There are no forms to configure and no modules to maintain. A worker describes what happened in plain typed language; Intero builds the case file and runs the investigation.
The case file follows a fixed procedure: intake → evidence → timeline → causal analysis → CAPA → review and sign. The steps can't be skipped or reordered, so every investigation is complete and auditable.
What the case file holds
Each part is built by the investigation as it runs, with nothing filled in by hand.
Incident record
The report and its classification (type, severity, likely OSHA-recordability, and contributing factors), captured the moment a worker types it in, with photos.
Evidence
Photos, documents, and accounts attached to the case. Intero requests the evidence an investigator would ask for, and names in the report what is still missing.
Witness accounts
Witnesses are treated separately. Where accounts contradict each other, the contradiction is reported as a finding rather than smoothed over.
Causal analysis
A causal chain at three levels (mechanism → proximate → systemic) in the right framework (5-Why, Fishbone, Bow-tie). Every finding cites the evidence it rests on.
CAPAs
Corrective and preventive actions linked to the specific causes they address, assigned to an owner by role and unit, with due dates and status.
Hazards & risk (HIRA)
Hazard identification and risk assessment with a likelihood–severity rating, so a finding can be recorded as a rated risk with its controls.
Recurrence
A new finding is matched against earlier similar findings across your organization’s history, so repeat patterns surface instead of hiding in separate reports.
Review & sign-off
A second AI pass rejects boilerplate; a human investigator reviews, edits, and signs. The signed report is frozen and exported to PDF. Intero advises; people decide.

Cited, always. Every finding points to the evidence behind it. Learned patterns can guide the investigation but are never cited as evidence.
Full audit trail. Every edit an investigator makes is recorded: the basis for the signed, frozen report, and the record of expert judgement.
Ask your library. Upload SOPs and standards and ask in plain language; answers are backed by the documents they cite.
Regulatory logs on demand. OSHA 300 and 300A generated from your incidents, clearly marked DRAFT for human review.
Roles only. Engine output refers to roles rather than individuals.
